The Certified Registered Nurse Practitioner is directly responsible to the collaborating physician and is a registered nurse prepared through a formal organized educational program which meets professional association guidelines to provide a full range of health care services including planning, organizing, coordinating and directing optimal patient care and outcome to assure quality patient care to the child, adult, and geriatric patient that meets regulatory requirements.
Essential Functions:
- The Nurse Practitioner may order the following in order to evaluate, diagnose, and treat the child, adult, and/or geriatric patient: Medications (as allowed by regulation and license), Diagnostic tests, Referrals and Consultations, measurements, monitoring, procedures, and treatments (as allowed by regulation and license).
- Performs interview health histories. Sources may include, but are not limited to: child, adult, geriatric patients and/or families, and records requisitioned from Grand View Hospital, other health care providers or other facilities.
- Performs physical exams.
- Perform virtual visits using electronic communication tools to assess, diagnose and treat if appropriate.
- Interprets data from the history, physical exam, and diagnostic tests to identify actual or potential patient problems and needs.
- Develops and implements plan of care for the patient.
- Monitors responses to care plan and adjusts plan accordingly.
- Collaborates with members of the Direct Care Team regarding plan of care.
- Educate and counsel patients on disease prevention & management, health promotion and healthy lifestyle choices.
- Provides formal and informal education to other health care providers as necessary and/or requested.
- Provides teaching, consultation, guidance, counseling and emotional support to patients and their families.
- Discuss advance care planning with patient/families as appropriate.
